What do you mean by "multiple selection criteria"? Have you seen the selection of d a look at the Form Builder components in the extension directory?
You can have form fields with multiple selections e.g. check box groups, or a '<select>' field set for multiple option selections. For example, see Multiple Selection "<select>" box and view the example presented ("Try it yourself" button).
